Biomechanical Evaluation of a Standalone Lumbar Interbody Cage with Integrated Screws

چکیده

INTRODUCTION: Spinal fusion techniques including anterior lumbar interbody fusion (ALIF) have been used to alleviate chronic low back pain and segmental instability. Supplemental fixation using pedicle screws, translaminar screws, facet screws, anterior plates or spinous process plates is often used to increase the stability of the ALIF construct to enhance fusion and prevent subsidence and migration of the standalone cage. However, these traditional fixation methods often require additional posterior surgery and/or increased anterior exposure leading to increased complications and poor clinical outcomes. ALIF devices with integrated screws provide the opportunity to provide stable fusion constructs, eliminating the need for additional exposure and potentially allowing for standalone use.

Spinal Decompression and Stabilization: Expandable PLIF Cages over TLIF Cages for Spinal Fusion

Study design: An in vitro biomechanical flexibility and fatigue test comparing two different lumbar interbody fusion cages using mono segmental lumbar spine specimens. Objective: To investigate and compare the stabilizing effect of a transforaminal lumbar interbody fusion (TLIF) cage against an expandable posterior lumbar interbody fusion (PLIF) cage. Method: Six intact human lumbar spine segme...

Comparative biomechanical evaluation a lumbar spacer with integrated screws

INTRODUCTION The goal of lumbar spinal fusion is to limit pain by decompression and removal of soft tissues, followed by the formation of a rigid bony union between adjacent vertebrae. The general consensus for a rigid fusion construct requires careful preparation of the intervertebral space and the creation of a rigid fusion construct. Anterior lumbar interbody fusion (ALIF) provides good acce...

Biomechanical evaluation of an expandable cage in single-segment posterior lumbar interbody fusion.

STUDY DESIGN Controlled laboratory study. OBJECTIVE To evaluate the biomechanical characteristics of a new expandable interbody cage in single-segment posterior lumbar interbody fusion (PLIF) using cadaveric lumbar spines. SUMMARY OF BACKGROUND DATA One of the popular methods of treating lumbar spine pathologies involves a posterior lumbar interbody fusion using bilateral interbody nonexpan...

Flexibility and fatigue evaluation of oblique as compared with anterior lumbar interbody cages with integrated endplate fixation.

OBJECTIVE This study was undertaken to quantify the in vitro range of motion (ROM) of oblique as compared with anterior lumbar interbody devices, pullout resistance, and subsidence in fatigue. METHODS Anterior and oblique cages with integrated plate fixation (IPF) were tested using lumbar motion segments. Flexibility tests were conducted on the intact segments, cage, cage + IPF, and cage + IP...

Biomechanics of Lateral Interbody Spacers: Going Wider for Going Stiffer

This study investigates the biomechanical stability of a large interbody spacer inserted by a lateral approach and compares the biomechanical differences with the more conventional transforaminal interbody fusion (TLIF), with and without supplemental pedicle screw (PS) fixation. Twenty-four L2-L3 functional spinal units (FSUs) were tested with three interbody cage options: (i) 18 mm XLIF cage, ...
